Allow configuration of session modes and parameters

Give users the ability to configure session behavior, such as:
- Session mode (e.g., agentic vs conversational)
- Context window handling
- Tool authorization policies
- Other ACP session parameters

This could be exposed via VS Code settings, per-model configuration, or a UI in the chat interface.
